Tests such as cerebrospinal fluid analysis for the 14-3-3 protein, real-time quaking-induced conversion (RT-QuIC) assay, and specialized MRI sequences including diffusion-weighted imaging require deliberate ordering by a clinician who already suspects prion disease. Without that suspicion, those tests are never requested.

  3. "Has an MRI been ordered with diffusion-weighted imaging sequences specifically?" Standard MRI protocols may miss the cortical ribboning and basal ganglia changes associated with CJD.
  4. "Would a cerebrospinal fluid analysis including RT-QuIC testing be appropriate here?" RT-QuIC has high sensitivity and specificity for CJD and is available at several reference laboratories in the United States.
  5. "Can you refer us to a neurologist at an academic medical center who has experience with rare dementias?" This is a reasonable and appropriate request, not an insult to the treating physician.

How to Pursue a Second Opinion

Seeking a second opinion is not a betrayal of your physician — it is a standard and respected part of managing a serious or uncertain diagnosis. When symptoms do not fit a tidy narrative, or when a patient's condition is deteriorating faster than the diagnosis would predict, a second set of expert eyes is not just reasonable; it is essential.

Start by requesting copies of all imaging, lab results, and clinical notes. Academic medical centers with dedicated prion disease or rare dementia programs — including centers affiliated with the National Prion Disease Pathology Surveillance Center — are well-positioned to evaluate complex cases.
